Cushing Syndrome

It is the syndrome associated with hypercortisolism. Cause can be from Pituitary, Adrenal, Exogenous (i.e external steroid intake) or from any cortisol secreting cancer. Symptoms may vary according to the site of production, however common symptoms include:

Truncal obesity, rounded face 90%
Hypertension 75%
Facial pletharo 75%
Hirsuitism, acne 65%
Thin skin, easy bruising, striae 65%
Proximal muscle weakness 60%
Oligomenorrhea/amennorhea 60%
Psychiatric disorder 40%
Oedema 40%
Osteoporosis 40%

Impaired blood glucose, loss of libido etc.

Screening test includes Screening test consists of 24 hr urinary free cortisol, Night time cortisol (Serum, salicary), Overnight low dose dexamethasone suppression test. Confirmatory test may include Low Dose Dexamethasone suppression test and ACTH.
Treatment options includes Adrenalectomy, Endoscopic Pituitary surgery or Removal of Extra-adrenal tumour.
